Description

American Traveler is seeking an experienced RN for a Case Management position at a Level III trauma community hospital, requiring a minimum of 5 years of experience including ED and acute hospital case management.

Details

• Work setting is a 226-bed Level III trauma community hospital serving a large rural region
• Assignment is in the Case Management unit, floating throughout the hospital as needed
• Patient population spans multiple units including ER, ICU, Med/Surg, NICU, Oncology, and Tele (no L&D)
• Daily caseload of 15–20 patients depending on acuity and staffing
• Schedule is 4x10-hour day shifts, 0700–1730
• Every other weekend rotation required
• No on-call responsibilities
• EMR: Epic (experience preferred)

Requirements

• Active WA or Compact RN license required
• BSN required
• Current BLS certification required
• Minimum 5 years of RN experience required
• ED and acute hospital case management experience required
• Strong assessment skills and experience with case management and discharge planning required
• Demonstrated ability to discharge patients with complex social circumstances (e.g., homelessness) to appropriate levels of care including home health, SNF, LTAC, IP, assisted living, or shelter

• Responsibilities include case management and discharge planning across a diverse patient population
• Must be flexible and a collaborative team player within a small case management team
• Scrub color requirement is any solid color or business casual attire
• At least one supervisor-level reference (Charge Nurse, Manager, Director, or above) required for consideration; a second reference will be needed prior to start
