American Traveler is seeking an experienced Home Health RN for a 13-week travel assignment in Lexington, KY, requiring prior Med/Surg hospital experience and proficiency with Homecare Homebase (HCHB).

Details

• Home health care setting serving an adult-only patient population
• Case management caseload of approximately 30 patients with a daily point target of 6.5
• Responsibilities include Start of Care (SOC), recertifications, and daily visits
• Monday through Friday, 8am–5pm, with occasional weekend and holiday shifts
• 8-hour shift length
• Weekend rotation frequency depends on the number of nurses in the rotation
• Homecare Homebase (HCHB) is the required EMR for this position
• No on-call requirements
• A phone and tablet will be provided for field use

Requirements

• Active KY or Compact (NLC) RN license — must be active at time of consideration; pending licenses not accepted
• Current BLS certification required; pending certifications not accepted
• Minimum 1 year of Med/Surg experience in a hospital setting
• Proficiency with Homecare Homebase (HCHB) is a hard requirement for consideration
• Driver's license required for consideration
• Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire

Additional Information

• Conduct home visits including SOC assessments, recertifications, and routine daily patient visits
• A clinical manager or educator is available 24/7 for emergencies and during business hours (8am–5pm) for general support while in the field
• Orientation consists of approximately 2 weeks, including supervised field visits with a work coach (approximately 12 orientation hours total)
• Holiday rotation coverage is required
• Scrubs required; no color restriction
