American Traveler is hiring a travel RN Case Manager for a Care Management position in Portland, ME, requiring 5+ years of experience in discharge planning and care coordination.

Details

• Day shift position, 7:30 AM – 4:00 PM
• Care Management department within a hospital setting
• Caseload ratio of approximately 1:20–24 patients
• Weekend hours are occasionally available but not required
• No on-call requirements
• Floating required to cover Care Management department needs, including a potential float to other facilities within a 25-mile radius
• 13-week contract

Requirements

• Active RN license — Maine (ME) or Compact license required
• Minimum 5 years of RN experience required
• Hands-on experience in discharge planning and care coordination required
• Driver's License required for consideration
• Two professional references required for consideration
• SSN and DOB required for consideration

• Primary responsibilities include discharge planning and care coordination
• Candidates with a flexible, solution-oriented, and calm demeanor are encouraged to apply

• Use of AirPods, laptops, and personal cell phones on the unit is prohibited
• Candidates residing within 90 miles of any MaineHealth facility in Maine or New Hampshire are considered local and may not be eligible for the standard travel pay rate
• Local travelers are not eligible for contract renewals or extensions
• Candidates who have worked at any MaineHealth facility within the past 12 months should disclose this at time of consideration
• Travel pairs or groups will not be considered for this position
