American Traveler is hiring an experienced RN Case Manager for an acute care hospital Case Management position in Florida, requiring a minimum of two years of recent acute care CM experience with discharge planning expertise.

Details

• Acute care hospital setting with a Case Management unit
• Adult and geriatric patient population with a 1:25–30 caseload ratio
• Day shift schedule: 5x8-hour shifts, 8:30 AM–5:00 PM, Monday through Friday
• One full weekend shift required every four weeks
• No on-call or holiday coverage required
• EMR platforms used include Epic, CarePort, AllScripts, and PowerChart — proficiency in all is required
• Microsoft Office Suite proficiency is required

Requirements

• Active FL RN license or compact RN license currently in hand
• Minimum of two years of recent, consecutive acute care hospital Case Management experience within the past 24 months in a US hospital
• Discharge planning experience required
• Level 1 Trauma experience preferred
• Proficiency in Epic, CarePort, AllScripts, PowerChart, and Microsoft Office Suite required
• Degree must be from an accredited school or from an institution approved by the Florida Board of Nursing (FBON)
• All certifications must be current and in hand at time of consideration
• Proof of vaccination status required — declinations are not accepted

Additional Information

• Responsibilities include completing initial patient assessments and revisiting patients at a minimum of every third day
• This is a smoke-free facility
• Local candidates within a 60-mile radius of the facility are considered, though at a different pay rate than travel candidates
• This facility is a Level 1 Trauma center
