American Traveler is hiring an experienced RN Case Manager for an inpatient case management position requiring a certified care manager credential, BSN, and recent Level 1 or comparable Level 2 trauma experience.

Details

• Inpatient hospital setting with a variety of care needs across units
• Daily caseload management across inpatient units
• Monday through Friday schedule, 8:00 AM to 4:40 PM
• No weekend or on-call requirements
• Documents and places orders within Epic EMR

Requirements

• Active NH or Compact RN license required
• BSN required
• Current BLS certification required
• Certified Care Manager (CCM) credential required
• Minimum 3 years of inpatient clinical experience required
• Minimum 1 year of care management experience required
• Recent Level 1 or comparable Level 2 trauma experience within the last 2 years required
• Epic EMR experience required
• 2 professional references from managers required for consideration

Additional Information

• Oversees daily caseload of inpatient patients, conducting initial assessments and facilitating individualized plans of care in collaboration with patients, families, caregivers, and the healthcare team
• Participates in interdisciplinary rounds to coordinate care
• Communicates with medical teams and patients regarding post-discharge needs including home health and inpatient rehab
• Interacts with post-acute care referral sources to establish discharge plans and places orders for home health, DME, and nutritional needs
• Candidates must reside more than 75 miles from the facility to be eligible — local candidates will not be considered
