American Traveler is seeking an experienced RN for a Case Management position in a hospital setting, requiring discharge planning experience and Cerner EMR proficiency.

• Hospital-based Case Management unit in Fort Wayne, IN
• Day shift: 8:00am–4:30pm
• Patient caseload ratio of 1:20
• Cerner EMR used; experience required
• Holiday coverage required for any holidays that fall within the contract period
• 13-week contract

Requirements

• Active IN or Compact RN license required
• BLS certification required; no certifications may expire within the first 30 days of the start date
• Minimum 4 years of experience in Case Management
• Discharge planning experience in a hospital-based care management setting required
• Driver's license required for consideration
• Candidates must reside more than 50 miles from the facility to be eligible

Additional Information

• Provides care management and discharge planning services for a hospital patient population
• Black scrubs or business casual attire required
• RTO requests must be less than one week in duration
• Candidates must have no gaps in work history of 90 days or more within the past 12 months
• Candidates who have worked at any CHS facility as permanent staff within the past 2 years are not eligible for this position
