American Traveler is seeking an experienced RN for a Hospice/Palliative Care position on a Supportive Care Team (SCT), providing field-based patient visits on evening shifts Monday through Friday in the Racine/Kenosha, WI service area.

Details

• Field-based hospice nursing role serving patients in the Racine/Kenosha, WI service area
• Supportive Care Team (SCT) unit within a hospice division
• Patient visits include PRN, routine, and death visits
• Monday–Friday evening shift, 4:00 PM–12:00 AM (8-hour shifts)
• 40 hours per week
• No on-call or call-back responsibilities
• No weekend rotation required
• No floating required
• Epic EMR

Requirements

• Active WI RN license or valid Compact RN license required
• Minimum 12 months of RN experience in hospice or palliative care, with most recent experience within the past year
• Epic EMR experience
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum of 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Perform field-based patient visits including PRN, routine care, and end-of-life/death visits
• Memorial Day coverage required (4:00 PM–12:00 AM)
• Candidates from the Continuing Health Patient Service Area (PSA) are not eligible for this position
