American Traveler is hiring a travel Care Manager II (RN or LMSW) for a Case Management position in Alamogordo, NM, requiring at least 2 years of clinical experience including acute care.

Details

• Case Management department within an acute care hospital setting
• Manages a diverse patient caseload from admission through discharge, including discharge planning, utilization management, and care coordination
• Day shift schedule: Monday–Friday, 8:00 AM–5:00 PM (5 days x 8 hours)
• 13-week travel contract

Requirements

• Active RN license (NM or compact) or active LMSW in the state of NM required; LBSW accepted for candidates with 5+ years of demonstrated success in a CHRISTUS Care Manager I role
• Minimum 2 years of clinical experience; at least 1 year in an acute care setting preferred
• Graduate of an accredited nursing school (BSN preferred) or Master of Social Work (MSW); equivalent experience in a CHRISTUS Care Manager I role for 5+ years may be considered in lieu of education
• Certification in Case Management preferred
• BLS (AHA) preferred
• Must provide a US Social Security Number for consideration
• 2 professional references required for consideration
• Active copies of all certifications required for consideration

Additional Information

• Collaborates with patients, families, physicians, and multidisciplinary teams to develop and implement individualized care plans from admission through discharge
• Monitors patient length of stay, identifies barriers to discharge, and works with the interdisciplinary team to resolve them
• Coordinates post-acute placement needs including skilled nursing, acute rehab, home health, hospice, DME, and community resources
• Serves as a patient advocate in navigating treatment decisions, end-of-life issues, and payor negotiations
• Participates in Multidisciplinary/Patient Care Progression Rounds and clinical performance improvement activities
• Addresses social determinants of health and assesses patients' formal and informal support systems
• Role involves movement throughout the hospital for the majority of the workday, with time also spent in an office setting
• Candidates must reside more than 50 miles from the facility to qualify for travel rates; local candidates are not accepted on this position
